The importance of volume control

This presentation was given by Nathan Levin from Mount Sinai Icahn School of Medicine. It was presented at the 1st International Dialysis Course in Senegal taking place December 5-7th, 2015 for which the theme was « Renal Replacement Therapy »  during Session 3: Hemodialysis.

This ISN webinar will outline peritoneal dialysis access methods, give a review of the literature in the field, discuss elements of dealing with complications and how to develop a PD access program.

This ISN webinar will present the relevance to modern clinical research of research principles established over the last 150 years; appreciate the importance of properly defining research questions and relevant research methods; understand the importance of collaboration with investigators with a wide range of research expertise; understand the principles of honesty and integrity with regard to data collection, analysis and dissemination; be aware of areas of both success and disappointment in recent renal research and recognise the many exciting opportunities for progress in modern renal research.

狼疮性肾炎(LN)是我国继发性肾小球肾炎中最常见的类型。其发生终末期肾病(ESRD)相关危险因素有性别、病程长短,肾活检时平均动脉压、尿蛋白水平、血清肌酐水平,以及是否存在贫血和低补体C4血症,此外随访过程中蛋白尿和血压的控制亦是影响ESRD的重要因素。不同病理类型的LN长期预后不尽相同,值得注意的是III型或IV型伴V型病变时,长期预后取决于增殖性病变。随着免疫抑制剂的应用,LN的预后得到明显改善,但仍存在一些问题,如完全缓解率低,反复复发,仍有10-20%患者10年后会进入ESRD,以及治疗相关的不良反应等。鉴于LN的致病机制,我们引入了多靶点疗法。多中心随机对照临床试验结果显示多靶点较传统环磷酰胺疗法可将完全缓解率提高20%,缩短到达缓解的时间(13周→9周),且不增加不良反应。针对循环自身抗体在SLE中的致病作用,在药物抵抗或疾病急性期药物还来不及起作用情况下,采用血液净化技术(血浆置换、膜式双重血浆置换、免疫吸附)快速清除致病性物质,达到缓解及减轻急性期器官损害的目的。目前该技术在血栓性微血管病、抗心磷脂抗体综合征、弥漫性肺泡出血、冷球蛋白血症等并发症中取得了一定的疗效。对于顽固性LN目前仍是国际性难题,我们尝试使用自体造血干细胞移植的方法使部分患者获得了缓解。

Introduction

Nephrotic syndrome is a chronic kidney condition that causes leakage of protein through the kidney to cause widespread tissue edema and increased thrombotic and infectious complications. Most children who develop the nephrotic syndrome experience prompt disease remission with corticosteroid therapy (prednisone or prednisolone) – although about 80% of children will experience one or more disease relapses and these relapses can continue into their adult life. Corticosteroid therapy is highly effective at inducing remission but has important negative implications for child health including slowed growth, obesity, impaired psychological well-being and longer term cardiovascular risk. The optimal dose and duration of corticosteroid therapy for children with nephrotic syndrome is uncertain because existing trials provide different results. In this Cochrane review from 2015, prednisone given for 2-3 months was as effective as longer term treatment (6 months) for preventing relapsing disease in children who have their first episode of nephrotic syndrome. Children with relapsing disease benefited from 5-7 days of daily prednisone therapy given at the time of an infection. The side effects of different treatment regimens are poorly understood.
