ISN-KDIGO-WONCA Joint Webinar on the CKD Early Identification and Intervention Toolkit
With the global burden of chronic kidney disease increasing, early identification and treatment is a worldwide imperative. To this end, the ISN-KDIGO CKD Early Identification and Intervention toolkit was developed to support kidney health professionals, primary care physicians (PCPs) and nurses working in primary care settings. This webinar will explore the toolkit through the perspective of the PCP, discussing CKD identification and risk stratification in the primary care environment. Panelists will also discuss how to increase collaboration between PCPs and nephrologists.
Learning Objectives:
- Understand the ISN-KDIGO toolkit elements and their uses.
- Understand how identification and stratification for CKD is done in a primary care setting.
- Provide insight into the tools from a primary care perspective and how they can best be applied in the primary care setting.
- Understand how to increase collaboration between PCPs and nephrologists.
