Primary care is often the first point of contact for people with emerging health concerns, making primary care physicians central to the early detection and long-term management of kidney disease. 

Early identification in these settings is critical, as many kidney diseases are diagnosed at later stages, while timely intervention can slow progression and improve outcomes. 

The ISN has developed a series of practical kidney care “cheat sheets” for primary care, turning complex guidance into clear, easy-to-use references that support earlier detection, better management, and improved patient outcomes.

Clear guidance on how and whom to assess, how to determine the underlying cause, and when to refer to help prevent progression and reduce complications. 

• Cardio-kidney-metabolic health 

A holistic approach to managing interconnected kidney, cardiovascular, and metabolic conditions, combining lifestyle interventions and guideline-directed therapies to improve long-term outcomes. 

• Kidney replacement therapies and conservative care 

Guidance on approaching kidney failure, including shared decision-making around dialysis, transplantation, and conservative care, aligned with patient preferences and quality of life.
